Upon entering the office, the sign in sheet will be in the reception area. The office member will provide you with new patient forms (which are also available online), for print and completion so that you may them with you, time permitting. Completed forms should be returned to the registration desk in your pre registration paperwork, where the office manage will assist you. Note: Should you need assistance in completing your forms, please do not hesitate to ask a staff member for help. Upon completion, the office manager will consult you regarding your completed documentation, insurance coverage, and payment options. Please do not forget to bring a legal photo identification and any insurance information, if any. Any children under the ages of 18, are required by law to be with a legal guardian before any Chiropractic care is to be given. Yearly, we schedule an appointment at registration to update your health insurance and other personal demographic information. Please inform our staff whenever there is a change in your health insurance coverage, mailing address, phone number, emergency contacts or local stay should you be in town temporarily,
Payments due from patients are collected at the registration desk. These payments could include: copayments, deductible fees for office visit, and self payment fees, supply fees, any therapeutic procedure fees.
There may be additional charges posted to your account after completion of your services. Patients or their guarantors are responsible for payment of all charges for services rendered, regardless of the amount covered by insurance companies or other third party payers.
